New to the forum.. 

 

This is something I came across today. Scientists from NIMH seemed to have been involved in the study. Too much hype? Or perhaps game changing for people with depression..?

Hype in my opinion. I can't exactly speak for clinical application but I can speak from my more wreckless days of insufflating tranquilizers and involving myself with others who did that nothing good really came out of it. I had pretty serious depression too. In my opinion treating depression with ketamine is about equivalent to treating depression with opiates. Sure you're going to feel better but it's not sustainable and there are considerable risks. For one, arylcyclohexylamines are known bladder irritants which can result in irreversible ulcerative cystitis with long term use. Anecdotally it seems that this effect decreases with potency making PCP a better choice from this angle as it's about 20x more potent. But even that isn't advisable. I'm not sure it's ever been proven which metabolite if any was directly responsible, so who knows if hydroxynorketamine would be any different but i doubt it based on what we know. Also if you've been around people who've used this stuff long term there is some real off stuff going on in the noggin. I'm the kind of person who will entertain shamanism and all that, but dissociative psychosis is a different breed of fucked up. Beyond that are descriptions of vague neurological problems from prolonged use. Probably not as much an issue with therapeutic use, but who knows at this point.

 

My opinion, if you want to treat recalcitrant depression with psychotropics serotogenic hallucinogens are the stronger candidate. Two ayahuasca ceremonies spread out about 3 months apart turned my life around after ten years of major depression. That was about 3 years ago. My life has only gotten worse but my attitude continues to get better and I can definitively place those changes on those two nights. In my opinion these are some of the strongest candidates for treating depression period, though care needs to be taken and a cultural understanding of proper usage needs to be developed. The ceremony was perhaps more important than the substance itself. Though I do think ketamine might have some value in treating acute depressive episodes. I recall reading about some interest in that and it seems viable to me. But i'd be shocked if they found any use for this in long term treatment
