Memantine, Wellbutrin, Adderall and supplements - need advice

I mainly read multiple forums and hardly ever post.  I like to research before asking questions but I've exhausted all my search options and need help. Any advice is appreciated! 

 

I struggle with binge eating and excessive exercise. I also have a hard time concentrating and getting things done. I constantly feel overwhelmed and struggle to even clean my house, respond to email, answer a phone call, or even attend social functions.  I put on weight easily and have always been on the heavier side.  Weight issues are a big deal for me.  I've had adrenal fatigue (HPA dysfunction) for years and it keeps getting worse.  I'm in what they call stage 3-4 adrenal fatigue.  My hormones are almost non-existent (progesterone, estrogen, DHEA, testosterone) and my thyroid is shot (low TSH, low free t3, low free t4).  I'm currently taking syntyroid and cytomel combo but my thyroid is still extremely low.  I have ADHD to some degree and was seeing a counselor for about 2 years. I'm exhausted all the time, can't concentrate and in addition, I'm injured and can't workout like I used to.  I do still workout but am in so much pain I can't run and my energy is so low I can't handle more than 20-30 minutes of cardio without being pooped out. I basically walk about an hour 4-5 days a week and weight train moderate-heavy weight three days a week.  I don't recover from workouts well so I've learned not to push it too hard. I work full time and while my schedule is somewhat flexible, it is very stressful and I work downtown which means driving about 30 minutes to and from work.  I walk to the park daily but certainly not very nature like for stress reduction. I'm pretty clean on my diet.  Mainly veggies, meat, and fat.  Little to no fruit. Yogurt is the only dairy I have however my food sensitivity test showed I'm highly reactive to dairy.  Starches on workout days but mainly low carb.  My insulin is really low according to blood work.  However, my high reactive CrP is high, WBC always low, temperature runs around 97, blood pressure low.  I don't have any autoimmune diseases or parasites. I've been tested for about everything with my functional medicine doctor.  I do chew gum and have a bit of artificial sweetener.  Stevia is my choice but do use products containing sucralose. I've read a lot about sucralose so comment if you'd like if it pertains to this discussion.

 

In the last 5-6 years I've experimented with many supplements in hopes to increase motivation, reduce depression, help weight loss, lower adrenal symptoms.  I started with tyrosine, DLPA and 5-htp to balance neurotransmitters.  I then started with others like ALCAR (love this stuff), piracetam and aniracetam (hated these), Phenylpiracetam (love), other choline supplements, herbs like rhodiola, ginsengs, mushrooms (chaga, reishi, cordyceps, lions maine), sulbutiamine, phosphatidylserine, theanine, provigil. I've also tried many diet pills and preworkouts through the years including those with DMAA. I was once using some chinese diet pills that actually worked for me.  Come to find out they contained sibutramine.  Now sibutramine  made me feel normal. I had to say that was one of the best products I've ever taken and it so sad that I can no longer obtain this. 

 

After being diagnosed with ADHD, I talked to my doctor about options.  ADHD is highly debatable in my mind.  Yes, I am somewhat all over the place but I also think hormones come into play here too but no doctor will address this with me.  I even went to a functional medicine doctor and shelled out $6000 for them to basically tell me what I already know.  I decided to go on Adderall XR after being very resistant due to it's long term side effects. As a long time researching on brain health, I knew this might not be the best option but I was willing to try anything for relief of the feelings I have.  I started Adderall XR 15mg December 2015 using it 2-3 days a week.  I love the way it makes me feel but hate the days I don't take it.  I was productive, had less stress, was nice to people, social, happy, able to focus and not feel overwhelmed by every little task. Before adderall I was to a point where I couldn't even pick up a dish in the sink without feeling overwhelmed and binging on food.  They days off adderall were not great though.  It was like a truck hit me and I sat on the couch all day.  I didn't help me lose weight either so after a few months I decided to stop.  Having adrenal fatigue, using adderall is probably a bad idea and I know that. I'm playing with fire here but am using it responsibly.  I have used it intermittently in 2016 maybe once or twice a month.  

 

I was worried about the tolerance even just being on 15mg and didn't want to up my dose when I read about memantine.  I started Memantine in May 2015.  After the initial brain fog and disassociation, I got my dose to 10mg where I stayed until December 2016.  I tried titrating up higher than 10mg but couldn't ever get out of the brain fog.  Despite the low dose, I noticed significant affects on my mood and ADHD symptoms.  It took about 2 months when I noticed I wasn't binging as much and my anxiety was reduced.  I must also note I started using a progesterone cream 500mg in January 2015 that I think also helped reduce my anxiety.  Over the last 6 months, my obsessive exercise behavior has dissipated which has helped my hormones and my menstrual cycle is coming back after 13 years of secondary ammenoreah.   Memantine has been amazing for my ADHD. I'm just not sure about the long term effects.  November of 2016 I starting using adderall xr again 2 days a week to get through some stressful times at work but again comes the days off adderall and I am very lethargic. 

 

Along with adrenal issues, I also have chronic high hamstring tendonosis.  That's another story in itself with the amout of money and resources I've spent trying to fix that which included prolotherapy, PRP, and peptides.  Last month I talked to my doctor about Wellbutrin to help the depression I feel from not being able to workout.  It is suppose to also help ADHD too and help with binge eating.  I started at 150mg and am now on 300mg.  I'm not sure what to think of wellbutrin so far.  At first I noticed I had a bit of a temper but that went away after 2 weeks. I've been on the 300mg dose for about 10 days and don't feel great.  I've been in a brain fog and feel super lazy.  I don't even want to workout which is totally unlike me!  I feel unmotivated to go to the gym or do anything physical.  The one thing I do like though is that I feel less OCD about exercise and food though.  I'm a little happier when I'm around people and don't mind socializing.  I usually have social anxiety but since starting wellbutrin that has gone down.  I find myself wanting to be helpful to others and laugh more.  I've only binged once since starting wellbutrin but that's because I was exhausted and wasn't listening to my body.  So my feelings are somewhat mixed.  

 

Now to the questions.....

 

1.  What's the protocol for using Memantine?  Can I use it long term?  I originally started using it for Adderall tolerance to keep my dose low but found it helped in other ways which is why I didn't stop using it.  My thought is that I should keep using mematine as long as I'm using adderall and only stop once I decide for sure to stop adderall. Otherwise, would it be safe to use adderall without memantine?  I get worried about downregulation.

 

2. Can memantine and wellbutrin be taken together?  I keep reading mixed things.  Some say they synergize eachother. Anecdotal testimonies say they don't work together.  Again, my mematine dose is only 10-12mg per day but I'm not sure if memantine is interfering with Wellbutrin's full potential for me.  I want to get the most out of wellbutrin but if memantine is having any sort of interaction that would prohibit Wellbutrin, I need to know this.

 

3.  To my understanding, the brain fog I get from wellbutrin is due to the anticholinergic effects it has.  Does this go away?  Would adding a choline source help?  I currently use ALCAR but that's all I can stand.  Any other choline supplement I take long term always makes me depressed.  I'm thinking this won't happen if I'm on wellbutrin but will counteract the effects of the dopamine.  Would it lower the effect of wellbutrin's dopamine effect?

 

4.  I currently use NAC for liver support hoping to support my T3-T4 conversion.  NAC is a glutamate modulator.  Would there be any interaction with NAC and memantine?  Would this increase or decrease any of the symptoms I'm having such as reduced motivation for exercise? 

 

5.  Would these other supplements I take interfere with anything?  

Morning: Tyrosine and/or DLPA 500mg (only when feeling low energy, never more because I'm already on a dopamine promoting prescription), Rhodiola, Green tea extract, NADH, PQQ, b-complex+B12, levothyroxine, liothyroixine, Xymogen T-150, memantine 10-12mg, wellbutrin 300mg, DHEA 10mg oral, xymogen adrenal support, Iodine

Pre-workout: Agmatine 500mg, citrulline 3 g, trimethylglycine 3g, caffeine 100-300mg varies, kre-alkalyn, cordyceps, BCAA,  tumeric+bioperine+coconut oil

Noon: Chorella, eleuthero 200-400mg, Ginseng, xymogen t-150, b complex, vitamin C, natural sources raw adrenal

Post workout: BCAA, Glutamine

Lunch: Fish Oil, multi, glucosamine, MSM, CoQ10, Vitamin K2, Vitamin E, Vitamin D, Gotu Kola,

Before Bed: melatonin 3mg, inositol 3g, magnesium glycinate 800mg, natural calm 2 Tbls, calms forte 1 pill, lemon balm 500mg, NAC 1200mg, milk thistle 150mg, Ashwaganda

Other: BPC-157 injected near the pubis ramis to help heal the tendonosis in the pelvis.  I've ready BPC helps upregulate dopamine but that's not why i'm using it. Would this injection interfere with memantine, adderall or wellbutrin?  Occasionally I use phenylpiracetam, sulbutamine, gluconolactone, DMAE, or higamine preworkout.  Of course never on adderall days.  Would any of those interfere with wellbutrin?

 

My overall goal is to be happy, motivated, get things done, able to focus be a healthy weight but not have excessive exercise and obliterate binge eating.  I've gain 20 pounds since I can't run from my injury and don't want to keep gaining.  I've never felt normal my entire life and always felt I have to work 100 times hard to achieve anything in life.  I'm trying to find the right combination for my brain and just want to make sure I'm making the right decisions.  I'm so desperate to lose weight I've even considered using zonegran but the side effects of zonegran seem so extreme just to lose a few pounds.  I would love any advice you have about my problems and appreciate answers to my questions above.

Thanks a lot for your time spent and writing down your thoughts. 

It's a little bit scary that you've tried so much stuff but still struggling with your symptoms, particularly strict diet.

Is it possible that you have your symptoms due to environment: a source of chronic stress like a job, family/coworkers/friends, polluted air in the city you live, toxins/mold in the place you live, noise or anything that disturbs your sleep? I believe there may be more variables which potentially would cause the symptoms like Irlen syndrome and sensitivity to particular colours and lights.

I hope that there is a light at the end of the tunnel. Farewell.
