Since nobody seems to want to answer I'll take a stab at it.
I think it's better with food, because I think carnosine can cause upset stomach.

I did take it without. I (hopefully temporarily) stopped using it due to probable histamine issues.
I'm still looking for a good beta-alanine source to replace it.

I thought you were supposed to take amino acids and small peptides on an empty stomach. That's what I've always done with carnosine. You may need to get a big dose in quickly in order to overwhelm the serum carnosinase that is waiting to hydrolyze it.

For optimal absorption, probably taken with water/juice on an empty stomach, but I don't believe it's strictly necessary. Human/animal studies have been done evaluating carnosine absorption after a meat-containing meal showing that it's substantially absorbed.
I'm currently taking 650mg twice daily. To saturate the carnosinase enzyme and start realizing benefit from carnosine it's necessary to exceed approximately 950mg in an average adult. Then you'd want to go over 950mg by some amount to raise carnosine levels, whether it be a small increase over this or a larger one. Otherwise, carnosine levles will only go up temporarily for up to a few hours and then recede back down to baseline after administration, and not provide sustained higher levels in plasma. The idea's been put forth by supplement researchers that short spikes followed by falls may be less healthy than aiming to maintain consistent levels throughout the day. This is also a goal with a good percentage of pharmaceuticals, and side effects are often less when levels are kept constant. After the carnosinase enzyme is saturated, this shouldn't be difficult. Carnosine will then be gradually eliminated in a fashion similar to most other supplements, through the urine and by slow metabolism in the liver.
