Maybe you had some sort of long standing acetylcholine issue?

Before the more popular(and marketable)monoamine theory of depression the acetylcholine theory of depression was one of the leading hypothesis, I'd post some references, but it seems that most of the reviews on that theory were published in the 1980's.

It has also be documented that acetylcholine deficiencies can cause symptoms similar to what you described (depression, brainfog which is often thought of as ADD, etc...). From what I've read, using a acetyl donor such as ALCAR (acetyl L-carnitine) could help you increase your body's acetylcholine levels if used in conjunction with a choline source. But, seeing as you said that combination made no improvement in your condition maybe you should try a different approach.

Seeing as piracetam helped you significantly, perhaps some sort of DRI antidepressant would help you (both produce sympathetic nervous responses). The other thing I would suggest is getting your appendix looked at since you say you get a lot of pain in your lower right abdomen.
