Feeling really sick.

Doctors are brain dead where I live so I imagine I'll get better responses from people on this site.

Background: I've been sick for over three weeks now. I'm not prone to being sick, and when i do get sick it's over quick, so this one lingering around is unusual. In the first couple weeks my mouth broke out with many canker sores (about seven) and even a cold sore. I've been achy, fatigued, wake up with cold sweats and in pain, have an inability to leave my house, etc. I'm feeling really overwhelmed and prostrated by this. I saw a doctor a little over a week ago at a clinic. He checked my temperature and blood pressure. He said I didn't have a fever and my blood pressure was normal so he told me to go home and not worry because it's nothing. I progressively got sicker. Then about 4 days ago I developed a terrible sore throat. Just breathing feels like I'm inhaling razerblades and swallowing is terribly painful. The symptoms also all got worse. I woke up in a cold sweat and unbearable body ache and throbbing pain from my throat so I went to the emergency room. I waited 5 hours then saw a doctor. He said my glads are swollen and I just had a sore throat and prescribed penicillin. I'm taking the penicillin and just getting sicker. My glands are really swollen and it's visible in a mirror. It's like two huge pockets developed on my throat. I cannot eat or drink water because the pain is unbearable, and lozenges, gargling salt water, etc are ineffective. My tonsil is white and red and on the left side at the back of my throat there's a few white patches.

Is this something like mono or tonsillitis or worse?

Did anyone do a throat culture? It does not sound like strep because of the slow and gradual rise of the sickness, but if it were me, I would get it tested just to be sure. Strep can move to liver and heart, bad news. If the whites of your eyes go yellow, sign of failing liver, go to the hospital immediately.

On the other side of things, antibiotics would also be effective on strep but not something viral.

Have you considered glandular fever? If you are unlucky enough to have this, you can expect to be sick on and off for months.

How old are you? Is there a chance you've contracted herpes? Some people get really sick with it and it sounds like you might be one of them.

It appears you have a herpes mouth
infection, usually HSV-1, though this
happens more in childhood. It may
recur in adulthood, though more often
in people with weak immune systems.

HSV-2 (genital herpes) infections in
the mouth are also possible, though
relatively rarer.

It may also be EBV.

" Symptoms can include fever, fatigue,
enlarged glands and sore throat. "


A course of acyclovir tablets should help
you, whether HSV-1, 2, or EBV.

Typically 200 mg capsules five times
a day for five days, or three 400 mg tablets
two or three times a day for five days.
These are adult dosages.

Ask your doctor to prescribe it for you.

Newer antiviral drugs like valaciclovir
and famciclovir are also effective, but
more expensive. Generic acyclovir
tablets are common these days.

This is not recurrent herpes. It is almost certainly primary herpes.

Here are a few of my notes from oral pathology about it:

Primary Herpes
2 to 4 mm vesicles
painful gingiva
all oral areas are involved
primary infection in adults more in the laryngeal area
lasts 2 to 4 weeks

Treatment
Abreva (OTC) - doesn't work well
Lysine (OTC) 500 mg - 2 to 4 tablets/ day - debatable utility
Xylocaine Viscous 2%
Citrus bioflavonoids and ascorbic acid tablets (Peridin-C)
Zovirax ointment 5% (Acyclovir) - doesn't penetrate epithelium much, reduces course of disease 1.5 days
-mixed with DMSO penetrates faster
Zovirax capsules (200 mg)
Denavir - Penciclovir 1%
Valtrex - look in drug book for regime
5% Acyclovir with 1% hydrocortisone

they used to say "never use steroids"
now they mix acyclovir with steroids and get better results

A throat culture should be able to identify a wide variety of bacterial/viral pathogens within a few days. A rapid-strep test can pretty quickly ID strep via antibody rxn. Mono can be pretty quickly ID'd via serology (looking for antibodies in blood). How long have you been experiencing the more severe symptoms (razorblade throat/cold sweats/aches)? What's your temp now?

There is no genital or oral herpes, this is a misconception. I believe HSV-1 is more common in general, mouth or genitals.

I'll try to answer most of the questions:

No throat culture and no blood work were done yet. It was a superficial scan from both physicians apart from the blood pressure and temperature check.

Glandular fever is mono right? I'm considering it.

I'm in my late twenties. I don't think it's herpes simplex 1. I manifest no symptoms and haven't been with a girl for a couple months since breaking up with my ex. I always had herpes simplex 2 - that's the cold sore one? But I've never manifested it since i was a kid so it must have been a sign my immune system was under a serious attack.

Right now it's just the sore throat that's really debilitating. It just seems to get worse. The body ache and pain is nothing in comparison. Is it normal for a throat infection to manifest with flu like symptoms for 3 weeks before actually causing a sore throat?

I was going to see my family doctor today and ask for blood work and a throat culture but got snowed in then fell asleep all afternoon (I've been sleeping a lot). Hopefully tomorrow I can get there.

This sounds a lot like a severe case of mono.

its good to get sick while young and healthy so you are more likely to survive and adapt thus get stronger then at an older age.

What you're experiencing has to do with old age and it is natural. Best way to combat it is with supplements and good diet and avoid bad things like alcohol etc I get sensitive to sound when sick.

Mental changes can make you sick. If you feel sick out of nowhere something is triggering your mentality differently which can trigger your immune system
