Just discovered that I am ApoE 3/4+ homozygous MFTHR and MAO-A R297R. What are my options?

So I just received my 23andMe results and it seems that I am cursed on multiple fronts:

 

ApoE 3/4+ homozygous MFTHR and MAO-A R297R + a few heterozygous mutations in COMT, VDR, MTRR, BHMT, CBS which makes my lipid profile and overall methylation (70% loss of function) and glutathione cycles dicey.

I am currently following non-ovo-lacto pescetarian, mostly plant-based diet to battle persistent scalp psoriasis but the above results seem much more pressing items to address.
 
I had my last Cholesterol panel back in March and since I am below, but close to, the upper limit of the normal range for TC and sub 40yrs old (33), the family doc stated that I can't  get Cholesterol testing prescribed. Same for Fasting glucose and Hb1ac.  :sad:

My current daily regimen includes 400mcg 5-TMHF+ 500mcg methyl B12+25mg P5P +500mg Niacin (to boost HDL).
I think I also get some folate from the quotidian veggies. 
My amateurish understanding is that too much Niacin effectively halts my, already impaired, methylation cycle. Is this correct?
What are my options, if so?
 
Given the above obstacles, what do you think would be the best course of action going forward?
Are supplements like SAMe/TMG/NAC/5HTP/Lithium suggested or to be avoided in my case?
 
I understand that saturated fats are to be avoided (is Coconut oil bad too?). How about Ghee?
I already use mostly EVOO (60%), Hemp oil (10%), MCT oil (15%), EVCO/Ghee (15%).

I am Homozygote for MTHFR 677t and am 55. This is really about methylation. Unfortunately, found out after 5x bypass. I have a VERY integrative cardiologist now. You should have a full lipid panel run to check your particle size and number along with your LPA. At least from a coronary perspective, it will give you a better picture. Here is what I am doing:

 

For MFTHR I use the Adenosylcobalamin B12 as it has less anxiety related effects. I also use Methylfoliate both at 1000mcg.

From Cholesterol of 240 to now 167 and LDL of 140 to now 67 I take in order of prescribing: Berberine 500mg; Citrus Bergamot 500; Pantethine 600mg all 2x a day; 1 10mg Zetia

 

I take 1000 niacin (time release) and Aronia Berry for lowering LPA; LPA is like putting glue on something already sticky; and a good probiotic

My diet has changed from no red meat but, a lot of chicken and Turkey (farm grown GMO Free) to a mostly plant based diet. Still have the other once a week or so but, Fish is included about 3x a week. Make sure its low mercury type.

 

If you start now with both the B supplements it should be ok with your niacin but, I'm no doc.. just someone who is catching up on years of not knowing my issues. Hope to learn more here and other threads

When you say full lipid panel - do you mean the NMR lipid profile?

 

Since the initial post, I upped the Methylfolate to 800mcg (+ having almost daily leafy greens adds more to this number).

B12 is still at 500mcg but I may bump it up to 1000mcg. P5P/B6 remains at 25mg.

Berberine at 400mg is also part of my daily regimen.

I take Pantothenic acid at 500mg for its purported skin/hair benefits so B5 is covered somewhat too. PABA too.

 

My Total Cholesterol dropped to 4.7 (180) after a year of the mostly vegan diet (with occasional fish) as per last year's blood panel. Will know again what is the current number in December at my annual physical.

On the niacin - my understanding is that the time-released/flush-free version is taxing the liver and does not confer the cholesterol benefits the instant-release nicotinic acid does which is why I stuck to the latter.

It is also taken away from the methyl donors ( MTHF, P5P, TMG) on the opposite end of the day as it acts as a methyl sink (counteraction).

I often take D-Limonene at 250mg - not sure if it does the same at the Bergamot extract - I will look into it.

66B CFU Probiotic is also a frequent addition to my daily intake.

 

Dried aronia berries powder is part of my cereal mix - I love it.

I do mean an NMR lipid profile.

 

My Niacin issue is really targeting LPA and my liver is tested regularly so far.

The B12 is specifically for the methylation issue so I guess my doc thinks that will offset anything regarding a Niacin issue.

There is a big difference between PAcid and just Pantothenie. While a derivative, it is not a vitamin but, has proven useful in lowering blood lipids. and triglycerides.

I get the Aronia capsules on Amazon.

 

Just had blood drawn today. Have hem check your LPA. Also, doc said avoid folic acid and coconut oil...

I was thinking about 23andme but I was worrying about it making me unecessarily worried about some gene mutation. Worrying is bad for you, right? Men in my family lived to 80s and women almost entirely to 90s I kind of think how bad can the genes be?

Basically I follow what my integrative cardiologist recommends. I'm sure there are some herbs that would help but, he is considered one of the top experts in lipids so its per above.

 

And everyone reacts differently

While not a herb, Sytrinol lowers cholesterol reliably.

Tried it. Doesn't do much of anything to my numbers. I'm trying a proteolytic enzyme combination and I'll know if that is helping in a couple of months when labs get rerun. Been reading a lot about this possibility..
