Visual Snow Syndrome / Persistent Migraine Aura Stack Help Please

Looking for some help creating a stack of supplements that has the best chance to help me.  I'm a 37 year old male in the USA.

 

Background - About 2 months ago I had an opthalmic migraine with the classical visual aura symptoms which lasted about 10 minutes.  The next day I had an episode that mirrored stroke symptoms and ever since I've had a persistent migraine aura.

 

Specific symptoms that have persisted: (24/7)

Visual Snow in entire visual field with eyes open or closed

Photophobia

Palinopsia

Entoptic Phenomena (Blue Field mostly)

Floaters

Halos around lights at night

Tinnitus of verying intensity and quality

 

The symptoms seem to improve during the day but every night when I wake up after sleeping more than 45 minutes or so they are much, much, worse again.  This pattern has persisted every night.

 

Interesting test results:

My blood CO2 is very low and outside the normal range.

My SED rate is 1, very low.

My factor 8 is high, outside the normal range (hypercoagulative blood)

My RBC count is at the very top of the normal range

Saliva and Urine PH are consistently acidic, between 5 and 6.5

Vitamin D3 was very low

BP, sugar, electrolytes are normal. 

 

Other Symptoms:

I have Postural Tachycardia (POTS)

I sometimes blackout (but not completely feinting) when I stand up if I have been active.

I have a history of kidney stones which I passed without treatment by a doctor

I have a history of gastric ulcers which perforated and required surgery (4 years ago)

I wake up every 90 minutes or so when sleeping.  I never ever ever sleep more than 2 hours without waking up.

About a week before the migraine attack I had a very infected wisdom tooth extracted.

 

Until the last few years I was very active, in shape, and was a top level competitive golfer, even considering a professional run. 

 

I've been to several doctors, and finally after much time and difficulty found one (a DO neurologist) at a top hospital who actually knows what persistent migraine aura is, and ordered tests which gave the above results.  My eyes are fine and CT and MRI showed no signs of infarction and routine EEG was normal.  But he's so busy I won't see him again until September and I'm considering 2 days of infusions designed for migraine pattern (cortical spreading) abortion, but the infusions include some powerful stuff: DHE, Depacon, steroids, Reglan, Toradol, and of course magnesium among others. 

 

I'm currently ramping up on Lamictal (Lamotrigine) and taking 25mg daily at present for the next couple weeks before it increases.  I'm also taking 25mg of amitriptyline which gives me the most vivid dreams you can imagine, though I still wake up frequently.

 

I'm taking Magnesium Citrate, B2, and CoQ10 to reduce the frequency or prevent migraines.  I'm also taking b6, b12, folic acid, and gingko to keep homocysteine under control and potentially dilate blood vessels, and 50,000 IU of Vitamin D3 once a week.  Doc says I should take a baby aspirin but I haven't started doing that yet. 

 

I'm thinking I need to add Uridine Monophosphate, Fish Oil, Vinpocetine, ALCAR, CLA, and Zinc, but I'd love to hear some thoughts.  Melatonin seems to make the problem worse.  Also considering GABA, Phosphatidyl Serine, Feverfew, St. John's Wort, L Tryptophan/5-HTP.  Also, paleo-diet?  Thoughts on that topic?

 

About this disorder, it appears from research to be related to hypermetabolism in the right limbic gyrus and left anterior cerebellum, and migraine is probably comorbid rather than causative.  My best guess because there is little research available is that this imbalance creates hyperexcitable electrical activity in the occipital cortex  Also before you ask, I have no history of hallucinogen use. 

 

If anyone read all that and has some thoughts, you are my hero.  In the meantime, continuing to research.

The best treatment for visual snow right now seems to be benzodiazepines. Although I don't usually recommend people take benzos, I think it will really help in your case, especially with your sleep. Klonopin every night, even a low dose of 0.5mg, will help you sleep and will very likely reduce your visual snow.

Tricyclics like Amitriptyline are pretty standard for migraines these days, though usually the go-to tricyclic is nortriptyline. 25mg of Amitriptyline is a small dose though, the standard is 100mg, and the max is 300mg. Perhaps an increase to 50mg would be helpful.

I really believe a benzo will go a long way in relieving your symptoms, so please consider it.
