Almost three weeks ago the doc changed my meds from instant released dextroamphetamine 2x10mg to 2x15 extended release (for ADHD-PI), and added 20mg citalopram for general anxiety. I told him that the d-amp fixed most if not all of my issues but the problem was that when it wore off or when I had a day off I still felt crappy. So he suggested adding an SSRI for the anxiety. We did, and I've been on 20mg citalopram since. I also got a prescription for 1mg tabs of lorazepam but they're not the point here since I haven't used them much.
The first few days on the new meds were great; the extended release d-amp really was working well. Then the citalopram started kicking in. Ever since, I'm feeling more and more lazy, apathetic, unmotivated, tired, inable to feel empathy, and general inability to "get it going". I have a harder time getting out of bed, something that I was already struggling with, and I really need to push myself to do the smallest things. I'm at times more anxious, at times less (more like not feeling anything at all, listless), but I'm a lot more depressed than before. Last night I was completely exhausted and went to bed at 10am and didn't wake up until 6.30am. Usually I go to bed around 1am and wake up to pee at least once or twice. Also the effects on libido aren't too much fun.
My next doctor's appointment won't be until after 9 days, since he is only in once a month.
What should I do? I switched from taking the citalopram in the morning to taking it in the evening 5 days ago but the effects are the same.
I know SSRIs take time to work and that the initial phase can make things worse but this is quite alarming to me.
My options as I see them now:
1. try to get an appointment with another doctor at the office earlier. my doc said I could do that if I wanted to but I have pretty good rapport with him and would rather see him than somebody else. And the difference in days might not even be that substantial

2. start coming off of it myself by lowering the dose to 10mg/day. I'm a little hesitant to do this too..

3. just wait it out until I meet my doc in 9 days. It's going to be tough but...

I was on sertraline for a month before 50mg/day and it was definitely not as harsh as citalopram. I stopped sertraline because I suspected it was causing my rash (which turned out not to be the case)

I also don't know what to suggest to him next time? Dextroamphetamine like I said pretty much takes care most of the issues I have but relying only on a stimulant especially on a daily basis is not an ideal long term solution IMO.

Thanks

Escitalopram made be incredibly tired but Fluoxetine fixed that, maybe it's as simple as switching the SSRI you are taking... I don't have D-AMP to go with it, though

How long were you on escitalopram before switching to fluoxetine? How long did it take for fluoxetine to start having an effect? Did you like fluoxetine in general?

I'm also noticing that I'm getting forgetful, missing tasks at work/school, something that's really out of character for me.

About 3 weeks of Escitalopram, in between I also tried Duloxetine but that was too anxiogenic so we ended up with Fluoxetine (a lot more activating than the other SSRIs, so possibly what you need). Best to discuss with your doc, I would say. I also found it quite hard to focus on the Escitalopram on some days, so that might be the case for you, too.

As for time delay, Fluoxetine was quite quick for me but it's definitely possible to use both during the cross over...

Ok thanks. I didn't know that about fluoxetine, I'll have to look into it. Always thought fluoxetine/prozac was the poster child for SSRI related problems, but I guess that's more of a cultural statement.
I might also going back on sertraline of which I have some left. I'll talk with the doctor though first.
I reduced my citalopram dose from 20mg to 10mg 2 days ago.

It's been 6 days since I switched from 20mg to 10mg a day in the evening. The negative effects are a little weaker i.e. I feel a little bit better/less worse. I'm seeing the doc in 3 days and will talk to him about getting off citalopram completely.

What was the verdict from your doctor, l-dee?

He agreed that I could go off it. At that time I had been on 10mg for a week, and haven't taken any since then. So I'm only on dextroamphetamine ER now. I'm still more forgetful and careless than usual. But it's getting a little better.
