Hello all,

First of all I did a short (2 month course) 30 mg per day of roaccutane 13 years ago (I'm 30 now) however back in the days this drug was limited only for serious acne so my doctor said 2 month course would be enough. I had mild persistent acne that was not responding to anything else. Anyway acne came back 1 month after now my doctor says it is because i didn't take the cumulative dose (at least 120 mg/kg)

Anyway for the last 12 years I tried to fix my problem naturally diet (gluten free, dairy free, low GI), supplements (a lot of them starting from vitaming D ending on nicotinamide riboside), antibiotics, topical retinoids, acne.org regimen, blue red light lamp and others. The only thing that helped was ketogenic diet but it only worked for 80% of my acne.

As for my acne itself it is only around my mouth, forehead and occasionally jaw line. Hard, painful, cysts. I'm 30 now so I believe this shit should be already over but it isn't. I did all the hormonal panels but the results came back in normal range. Same goes for food allergy panel.

What definitely breaks me out:

- carbs in every possible form and even smallest amount

- ejaculation (this may sound funny but it isn't)

This brings me to my last resort -  accutane which I have successfully declined for the last couple of years due to the potential side effects. But right now I'm depressed as fuck and I have no other thing left.

So my questions are:

- what can I take with accutane to reduce the possibility of having bad/permanent side effects especially IBD and erectile dysfunction?

- is it safe to take accutane with imao (moclomebide)?

PS. I was diagnosed with maldigestion syndrome due to damaged villi in the gut but my gastrologist said acne is not really connected to this problem. Even though I have been on the gut restoration protocol for a year (l-glutamine + probiotics + collostrum) -- it didn't help at all.

Guys are there any more tests I can do to find my cause of acne (MRI / CT scan or whatever)? Or give up already and take this damn accutane.

No idea about the Accutane sorry, but what cured me of my adult acne was to avoid all skimmed milk products and to take 100mg of Niacin (has to create a flush) and 15 mg of Zinc Picolinate daily

Accutane (isotretinoin) doesn't cause bowel disease (see PMID:  25207875 and PMID:  24535094) and any evidence of involvement with erectile dysfunction is sparse at best (and keep in mind this drug has been in use for 30+ years now).  If I were in your shoes, I'd stop messing around and get going with the isotretinoin.

I think there are a couple of academic papers that state something different, like:

http://www.nature.co...jg2010124a.html

http://www.nature.co...jg2006292a.html

some FDA reported cases:

http://www.fda-repor...m/accutane.html

regarding the ED I think you can find entire forums online where people try to resolve their ED problems caused by accutane.

There are some reported medical cases:

http://www.ncbi.nlm....pubmed/16447596

Sure the risk seems to be low but last time accutane triggered my MPB and the probability was 1:10000 cases.

I'm not saying I will not take it I just want to be sure I did everything I can before going this route again.

Early reports and small studies suggested a possible link with bowel disease, but the newer larger and better designed studies have shown not only no increased risk but a possible protective effect. People with severe cystic acne are already in a pro-inflammatory state and are therefore at a higher risk of other inflammatory/autoimmune conditions including crohns and ulcerative colitis. I don't doubt there are forums where people blame all kinds of things on isotretinoin, but like I said, any real evidence for erectile dysfunction is sparse. If it were a real phenomenon imo, we would have heard a lot more about it by now, after 30+ years and countless thousands of patients. Even the alleged link with suicide/depression has been pretty munch debunked by 5 or 6 different studies in the past few years.

Accutane can destroy lives. It absolutely ruined mine and nearly killed me. If you feel horrendous for whatever reason months or years after taking it then congrats. Accutane has done it's damage. Ever wonder why seemingly every person in the world who has taken Accutane comes down with serious health problems after?

Of all the extremely serious health conditions in the world you are far better off having acne. I would willingly give both my legs and one arm right now to have my health back from the immense damage Accutane has caused me. My hormones are through the floor and intense depression and anxiety which has only somewhat been dealt with after so many years have passed.  Why on earth would anyone take such a dangerous drug with all the new information we have available now?

Accutane is an anti-androgen. It screws with your hormones. It can help eliminate acne for extended periods of time just like it can help kill your Testosterone levels and pituitary function for extended periods of time.

Want acne relief? The best Cure I have ever found is coalface facial scrub from http://www.lush.com          . Forget all the worthless cleansers at the local drug store and endless creams.  This does it in just a matter of days.   If only the entire population knew about Lush Coal Face .  Imagine how many lives would be saved. There are so many parents putting their kids on Accutane and then wondering why in hell they are getting depressed or coming down with serious health conditions after the fact. It sickens me to no end.

Accutane was recalled off the market in the United States 4 or so years ago with 25 million awarded in Class Action Lawsuits. Additionally it was recalled in 10 other countries.     When will people connect the dots?

Go to a local lush location and ask for a free sample.   Nothing gets rid of acne naturally the way this does.   If there are no locations near you then you might as well just order. Better than slowly killing yourself with Accutane.

  http://www.drugwatch...ane/lawsuit.php Accutane Verdicts

Roche has been ordered to pay more than $53 million to Accutane patients so far, but is appealing many of the verdicts. This total comes from several large judgments:

  • In 2008, a jury awarded a woman with inflammatory bowel disease $10.5 million in compensatory damages and $78,500 for medical expenses. She began taking Accutane at age 12 and was diagnosed with ulcerative colitis at age 14, but the judge said there was not enough evidence for punitive damages.
  • In February 2010, the manufacturer was forced to pay more than $25 million — the largest Accutane award to date — to Andrew McCarrell, who developed inflammatory bowel disease years after he had taken Accutane. As a result, he had five surgeries and was forced to have his colon removed. The Honorable Carol E. Higbee, the judge who ruled in the case, said that McCarrell’s “testimony and that of his wife and doctors presented a picture of probably the worst case of pain, suffering and loss of quality of life I ever heard described in my eighteen years on the bench.”
  • In June 2012, a New Jersey court ordered Roche to pay $18 million in compensatory damages to two Accutane patients who had developed ulcerative colitis. The cases of two other Accutane users were also tried at the same time, but they were dismissed.

The tragedy is that I've seen patients with horrible scarring cystic acne all over their face who are now quite literally scarred for life because they (or their parents) got on the internet and read scare-mongering accutane horror stories.  Answers in bold below.

Accutane can destroy lives. It absolutely ruined mine and nearly killed me. If you feel horrendous for whatever reason months or years after taking it then congrats. Accutane has done it's damage. Really??  That's an interesting claim.  I've prescribed isotretinoin to 100+ patients (likely far more, but I don't keep a tally) over the past 7 years, and none of them has ever complained to me about any long-term health consequences (other than their acne clearing up).  Maybe Accutane had lasting effects on you  (or more likely your issues were caused by something else), but this is far from the norm in any case.

http://www.ncbi.nlm....pubmed/21103844

"Isotretinoin influences pituitary hormone levels in acne patients"

Interesting, but doesn't show any lasting/permanent changes and in fact says that the hormone changes are likely *beneficial* for acne during therapy.

http://www.ncbi.nlm....pubmed/19501473

-side effects of Accutane can remain after treatment stops

This is paper published in the journal Medical Hypotheses, so this is literally a hypothesis paper, and it doesn't even once mention isotretinoin

Accutane was recalled off the market in the United States 4 or so years ago with 25 million awarded in Class Action Lawsuits. Additionally it was recalled in 10 other countries.     When will people connect the dots?

Accutane was never recalled.  Roche voluntarily stopped making the branded form of it due to bad press, and instead there are now multiple other branded names of isotretinoin (Claravis, Amnesteem, Absorica, etc).  Isotretinoin is actively and widely prescribed.

Trial lawyers jumped all over the initial reports of a possible association of isotretinoin and inflammatory bowel disease, and won lots of money for themselves and clients.  Since then, much better studies have shown the association to be false.  Interesting article here:

Am J Gastroenterol. 2014 Apr;109(4):570-1

Isotretinoin and inflammatory bowel disease: trial lawyer misuse of science and FDA warnings.

Abstract

Based on the Food and Drug Administration Adverse Event Reporting System (FAERS), the FDA and Hoffman La Roche issued warnings of a possible causal association between isotretinoin and inflammatory bowel disease. While scientists studied the association, trial lawyers used the courts to award large sums of money to plaintiffs despite the absence of clear scientific evidence of a causal effect. In this Issue of the Journal, a well-designed, large pharmaco-epidemiologic study shows no association. The story of isotretinoin highlights the problems that occur when the FAERS is used in litigation prior to further study and scientific analysis.
